Medicover Genetics 4th DNA essay competition winners 

We’re thrilled to share the results of the Medicover Genetics essay competition for high school students as well as the two winning essays. This competition, held in celebration of World DNA Day on April 25th, offered students the opportunity to explore the significance of DNA and its impact on science and society. 

The subject of this year’s essay competition was: “Advances in Genetics Have Transformed Prenatal Care: Why Non-Invasive Prenatal Testing (NIPT) Matters in Low- and High-Risk Pregnancies?” 

The scope of this initiative was to raise awareness about the importance of DNA and its role in prenatal medicine, with particular focus on Non-Invasive Prenatal Testing (NIPT) and its contribution to modern prenatal care.  

Below you can find the two winning essays. The first-place essay is in Greek.

Όταν μια γυναίκα μαθαίνει ότι είναι έγκυος, μαζί με τη χαρά γεννιούνται και ερωτήματα: Είναι το μωρό καλά; Υπάρχει κάποιος κίνδυνος; Η σύγχρονη Γενετική μπορεί σήμερα να προσφέρει απαντήσεις νωρίτερα και με μεγαλύτερη ασφάλεια από ποτέ. Στο επίκεντρο αυτής της εξέλιξης βρίσκεται ο Μη Επεμβατικός Προγεννητικός Έλεγχος (Non-Invasive Prenatal Testing – NIPT).  

Τι είναι το NIPT 

Το NIPT βασίζεται στην ανάλυση μικρών τμημάτων DNA που κυκλοφορούν στο αίμα της εγκύου. Ένα μέρος αυτού του DNA προέρχεται από τον πλακούντα και αντανακλά το γενετικό υλικό του εμβρύου. Με μια απλή αιμοληψία, συνήθως μετά τη 10η εβδομάδα κύησης, είναι δυνατόν να εκτιμηθεί αν υπάρχει αυξημένος κίνδυνος για συγκεκριμένες χρωμοσωμικές ανωμαλίες [1].  

Η μέθοδος παρουσιάζει πολύ υψηλή ευαισθησία, ιδιαίτερα για την Τρισωμία 21 (Σύνδρομο Down), ξεπερνώντας το 99% σε μεγάλες μελέτες [1]. Ταυτόχρονα, αποφεύγεται ο μικρός αλλά υπαρκτός κίνδυνος αποβολής που συνδέεται με επεμβατικές εξετάσεις, όπως η αμνιοπαρακέντηση [2].  

Ωστόσο, το NIPT αποτελεί εξέταση προσυμπτωματικού ελέγχου (screening) και όχι διαγνωστική εξέταση [2]. Το αποτέλεσμα εκτιμά την πιθανότητα μιας ανωμαλίας, αλλά δεν την επιβεβαιώνει. Η αξιοπιστία επηρεάζεται από παράγοντες όπως το ποσοστό εμβρυϊκού DNA στο μητρικό αίμα, ενώ η πιθανότητα ένα θετικό αποτέλεσμα να ανταποκρίνεται πράγματι σε πρόβλημα εξαρτάται και από τον αρχικό κίνδυνο της κύησης [2].  

Ποιες παθήσεις μπορεί να ανιχνεύσει;  

Το NIPT στοχεύει κυρίως στην ανίχνευση συχνών χρωμοσωμικών ανωμαλιών, όπως:  

  • Τρισωμία 21 (Σύνδρομο Down)  
  • Τρισωμία 18 (Σύνδρομο Edwards)  
  • Τρισωμία 13 (Σύνδρομο Patau)  
  • Ορισμένες ανωμαλίες των φυλετικών χρωμοσωμάτων (π.χ. Σύνδρομο Turner) [3]  

Η έγκαιρη αυτή πληροφορία επιτρέπει στους γονείς να λάβουν ενημερωμένες αποφάσεις και βοηθά τους γιατρούς να οργανώσουν κατάλληλα την παρακολούθηση της κύησης ή την παραπομπή σε εξειδικευμένες μονάδες, όταν χρειάζεται [3].  

Γιατί είναι καθοριστικό σε κυήσεις υψηλού ρίσκου;  

Σε κυήσεις υψηλού ρίσκου — όπως όταν υπάρχει αυξημένη μητρική ηλικία ή ύποπτα ευρήματα στον υπέρηχο — το NIPT λειτουργεί ως αξιόπιστο εργαλείο εκτίμησης κινδύνου [2]. Ένα αποτέλεσμα χαμηλού κινδύνου μπορεί να μειώσει την ανάγκη για άμεση επεμβατική διερεύνηση, ενώ ένα αποτέλεσμα υψηλού κινδύνου καθοδηγεί σε διαγνωστική επιβεβαίωση [2].  

Με αυτόν τον τρόπο περιορίζονται οι άσκοπες επεμβάσεις, χωρίς να μειώνεται η ιατρική ασφάλεια εκεί όπου απαιτείται.  

Γιατί έχει σημασία και σε κυήσεις χαμηλού ρίσκου;  

Το «χαμηλό ρίσκο» δεν σημαίνει «μηδενικός κίνδυνος». Επειδή οι περισσότερες εγκυμοσύνες αφορούν γυναίκες κάτω των 35 ετών, αρκετές περιπτώσεις χρωμοσωμικών ανωμαλιών εμφανίζονται αριθμητικά σε αυτή την ομάδα [3].  

Για παράδειγμα, μια γυναίκα 32 ετών χωρίς άλλους παράγοντες κινδύνου μπορεί να θεωρείται χαμηλού ρίσκου. Παρ’ όλα αυτά, το NIPT μπορεί να της προσφέρει μεγαλύτερη βεβαιότητα και να μειώσει το άγχος που συχνά συνοδεύει την αναμονή των αποτελεσμάτων.  

Σε αυτές τις περιπτώσεις, το NIPT μειώνει τα ψευδώς θετικά σε σχέση με τον παραδοσιακό έλεγχο και περιορίζει την αλυσίδα επιπλέον εξετάσεων [3]. Γι’ αυτό και διεθνείς κατευθυντήριες οδηγίες αναγνωρίζουν ότι μπορεί να προσφέρεται σε όλες τις εγκύους, ανεξαρτήτως αρχικής κατηγορίας κινδύνου [3].  

Πώς συμπληρώνει — και δεν αντικαθιστά — τις συμβατικές μεθόδους;  

Το NIPT δεν αντικαθιστά τον υπερηχογραφικό έλεγχο, ούτε τις επεμβατικές διαγνωστικές εξετάσεις όταν υπάρχουν σαφείς ενδείξεις [3]. Το σύγχρονο μοντέλο προγεννητικής φροντίδας βασίζεται σε συνδυασμό υπερηχογραφικού ελέγχου, NIPT και διαγνωστικής επιβεβαίωσης όταν χρειάζεται [3]. Παράλληλα, το NIPT έχει και περιορισμούς. Σε σπάνιες περιπτώσεις, παράγοντες όπως χαμηλό ποσοστό εμβρυϊκού DNA ή βιολογικές ιδιαιτερότητες του πλακούντα μπορεί να επηρεάσουν την ακρίβεια του αποτελέσματος [2]. Γι’ αυτό η σωστή ενημέρωση και η γενετική συμβουλευτική είναι απαραίτητες.  

Συμπέρασμα  

Ο Μη Επεμβατικός Προγεννητικός Έλεγχος αποτελεί ένα χαρακτηριστικό παράδειγμα του πώς η γνώση του DNA μετατρέπεται σε εργαλείο πρόληψης και φροντίδας. Προσφέρει ασφάλεια, αξιοπιστία και έγκαιρη πληροφόρηση, τόσο σε κυήσεις υψηλού όσο και χαμηλού ρίσκου.  

Σε έναν κόσμο όπου η πληροφορία μπορεί να προκαλεί σύγχυση, το NIPT προσφέρει κάτι ανεκτίμητο: τεκμηριωμένη γνώση την κατάλληλη στιγμή. Και όταν η γνώση συνδυάζεται με σωστή καθοδήγηση, μετατρέπεται σε δύναμη — δύναμη για υπεύθυνες, συνειδητές και ανθρώπινες αποφάσεις.

Second Place essay by Mariam Ezzeddin Massud Abdulkarim, Pascal English School Lefkosia, Nicosia, Cyprus  – NIPT – Non Invasive Prenatal Testing  

Imagine being able to learn information about your baby’s health early in pregnancy, without putting either the mother or the baby at risk. This is now possible through NIPT, non-invasive prenatal testing, a blood test that is transforming prenatal care for both low and high risk pregnancies.  

NIPT is a screening test done using the mother’s blood, usually from around 10-11 weeks of pregnancy. It works by analysing small fragments of DNA, called cell-free DNA (cfDNA), that are found in the blood. These fragments come from both the mother and the placenta. Because the placenta has the same genetic material as the baby, this allows doctors to screen for certain chromosomal conditions without needing to examine the womb. 

The test is used to detect conditions such as Down syndrome, Edwards syndrome and Patau syndrome. These conditions happen when there is an extra copy of a chromosome and can affect a baby’s development and health. Detecting them early in pregnancy gives parents more time to understand the situation and consider their options, as well as allows doctors and other healthcare professionals to plan appropriate care.  

One of the biggest advantages of NIPT is that it is non-invasive. Unlike procedures such as amniocentesis, which includes inserting a needle into the uterus and carry a small risk of miscarriage, NIPT only requires a simple blood sample from the mother. Statistics show that 1 in 200 women that have amniocentesis face a miscarriage. Amniocentesis can only be taken from 15 And 20 weeks of pregnancy, which gives the parents less time to consider their options and understand the situation. This makes NIPT a safer option while also giving highly accurate results. It can also be used in twin pregnancies, making it accessible to a wider group of pregnant mothers. 

NIPT is specifically useful in high-risk pregnancies, where there is already a chance of genetic conditions. This includes older mothers, those with a history of genetic disorders, or pregnancies where earlier screening tests have shown unusual results. In these cases, NIPT can provide more reliable information and help determine whether further diagnostic testing is needed.  

However, it is not limited to high-risk pregnancies, even in low-risk pregnancies, NIPT can be taken. Chromosomal conditions can occur unexpectedly, often, without any family history. Because NIPT is safe and simple to perform, it can be offered more widely, helping to identify potential conditions early or provide reassurance when results are normal.  
Even with all its advantages, NIPT does have limitations. It is a screening test rather than a diagnostic one, meaning it cannot provide a definitive result. Positive findings must be confirmed through invasive procedures such as amniocentesis. 

In rare cases, differences between the DNA of the placenta and the baby, known as confined placental mosaicism, can affect the accuracy of the results. In addition, NIPT does not detect all genetic or non-genetic conditions and may not be suitable in certain situations, such as multiple pregnancies beyond twins or in cases where the mother has specific medical conditions, such as cancer, which can release additional DNA into the bloodstream, or a history of blood transfusions or organ transplants, which may affect the accuracy of the results. 

NIPT is also part of a wider system of prenatal screening and is not used on its own. It is usually combined with ultrasound scans and other early blood tests to give a more complete picture of the baby’s development. For example, ultrasound can detect physical abnormalities, while NIPT focuses on chromosomal conditions. When used together, these methods help healthcare professionals to determine risk more accurately and decide whether further diagnostic testing is necessary. This combination of tests helps ensure that pregnancy care is more informed and balanced.  

Finally, Non-invasive prenatal testing has significantly improved prenatal care by providing a safe and effective way to screen for genetic conditions early in pregnancy. It is used in all pregnancies, for better-informed decisions, better clinical management and reassurance for expectant parents.
